To submit by mail: Please fill out the form and mail to: The Leukemia & Lymphoma Society, 333 East Carson Street, Suite 441 , Pittsburgh, PA 15219  ATTN: Team In Training.

To submit by email: Please complete form through Adobe Reader and click the "Submit by Email" button in the top right of the screen.  Select your email program, and then click "Send" in your email program.

Please keep in mind that when you sign up for Team In Training, you are agreeing to raise a minimum amount of money, specified by event on the first page of the registration form.  There is a $50 registration fee required to register that will go towards your initial supplies, training manual, team shirt, etc.  This fee can be paid by check or credit card and will go towards your fundraising minimum.
