Donation Submission Forms- When you receive donations by check or in cash, you'll need these forms whem submitting donations to LLS.
Alaska Airlines Mileage Form- For donations of $100 or more, Alaska Airlines will match donors' gift amounts at 1 mile per dollar.
Fundraising Letter Insert- Personalize and include this insert with your fundraising letters. It makes it easier for donors to choose an appropriate donation amount and makes the process easier too.
Fundraising Business Cards Teamplate- Customize these cards and carry them with you. When you speak with someone about what you are doing and why, hand them one of you cards with your fundraising URL on it!
Corporate Sponsorship Packet- Present this packet to companies. LLS/TNT offers many valuable incentives to companies for partnering with us and their sponsorship amount is credited to your fundraising efforts! Contact your staff person with any questions on corporate sponsorship.
Personal Recognition Sponsorship- Does one of your donors have someone they wish to honor by having their name printed on our event jerseys? With a $1000 personal recognition sponsorship, TNT will do just that. Every participant on our team will complete the event with the honoree's name on their back!
LLS' 501(c)(3) Confirms LLS' non-profit status and includes our tax ID number.
In-Kind Donation Letter- Use this sample letter as a template to request in-kind donations (non-monetary donations).
In-Kind Donation Form- Use this form for in-kind donations. Participants complete their portion of the form and include it with the procurement letter above when asking for in-kind donations. The donor completes their portion of this form and retains it for their tax records.
